Medical Leadership

Medical Director

Co-certifies the terminal prognosis with the attending physician and leads our interdisciplinary care team in developing individualized care plans. Provides consultation with other physicians to coordinate comprehensive medical care.

Attending Physician

Co-certifies the terminal prognosis with our medical director, assesses needs, and directs the customized plan of care with the hospice care team. Manages symptoms and prescribes treatments focused on comfort.

Nursing Care

Registered Nurse

Provides compassionate end-of-life care focused on comfort, dignity, and quality of life during the patient’s final months. Assesses patients, develops care plans, administers medications, educates patients and families, and offers psychosocial and spiritual support.

Personal Care Support

Hospice Aide

Provides personal care and hygiene for the hospice patient while offering light housekeeping services. Serves as a key point of communication with the care team regarding patient needs and provides essential family support during daily care routines.

Nursing Care

Provides compassionate end-of-life care focused on comfort, dignity, and quality of life during the patient’s final months. Assesses patients, develops care plans, administers medications, educates patients and families, and offers psychosocial and spiritual support.

Chaplain

Professionally assesses the spiritual needs of the patient’s family and provides emotional support and spiritual counseling. Assists with memorial preparations and offers guidance that respects individual beliefs and traditions.

Extended Care Team

Bereavement Coordinator

Develops care plans designed to meet the emotional needs of survivors for 13 months following their loss. Provides grief counseling and healing resources while planning annual memorial services for families.

Volunteers

Professionally trained to provide companionship and support for patients and their families. Offers non-medical respite time for caregivers and remains available for comfort and support during the time of loss and throughout the bereavement period.

Equipment Services

Ensures durable medical equipment necessary for patient care is 100% covered, including hospital beds, oxygen supplies, and mobility aids that enhance comfort in home or nursing home settings.

Medications

Provides formulary medication necessary to manage symptoms and pain associated with the hospice diagnosis, eliminating financial barriers to essential comfort care.
